put those fabric softeners for the dryer between your sheets for about a week. and if all else fail fabreeze works for a little while.
Take it outside. Spray both sides with Lysol or some other disinfectant spray. Leave it out all day to air, turning it every hour or so. Then spray both sides with Febreeze or your favorite odor cover-up. You may need to do this daily for 2-3 days, depending on how bad the smell is.
Buy a new one. ;)

You know what... Baking Soda works miracles. I fill my ashtray with baking soda, and when you walk in the house you won't be able to detect if a smoker lives there or not. It completely eliminates odor.

You most likely Never will. Smoke goes into the fibers andalmost imposible to remove.. I say Dump and Buy a new one ..Stop smoking if you smoke, it gets into the carpet, on the walls and turn a yellow over a period of time, good luck.

If you do not want to buy another because the kind you have is expensive...then I would say...pay a professional steam cleaner to do it, Its like getting your mattress washed and dried all at the same time.Than air it out for 2 weeks...
