There are alot of factors that will determine your cost. If you are planning on doing the job without a general contractor. I recommend that you get estimates from several different subcontractors

depends what you go with, one could go brick verses vinyl, one could go with hard wood floors verses carpet or linoleum/one could go with basic trim verses all decked out with crown molding popular trim, getting a room or building, getting it under roof is just a drop in the bucket to how one finishes it out
It depends totally on the scope of the work being done.
As everyone else has said it totally has to do with the scope of work. How much work you are doing yourself, who you are hiring for other work, if there is an architect involved, what materials are used etc. As a GENERAL rule of thumb in our architecture office in Michigan where we do lots of renovation/additions is $160 per square foot. That is for the average home with average materials. This can go up or down based on your area and the cost of lumber at that point. With renovations/additions I highly recommend that you enlist the assistance of an architect or custom home builder. Because you are addiing on to existing exterior walls, you may be affecting the load bearing capacity of the walls... so don't go hacking away. We typically charge around $4 per square foot for a professional design and full set of construction documents. Find a reputable designer in your area and get help! Good luck, happy renovating.
How high is up? Your question needs refining. Prices vary from region to region, and even from city to town. It costs more to work in places that are hard to get to (think Boston traffic) or in communities that are overly restrictive(think the Peoples' Republic of Cambridge). Give me a job in a town where I can leave a dumpster and park 4 trucks, without having to hunt for a parking spot. Also, warm-weather areas have different building codes than places that get 3 feet of snow. This means the cost of building in Virginia Beach and Augusta Maine are quite different.
