Are you building the addition?
If so a home owner can pull the permit at the local building department. You will need a set of plans for them to review so they can make sure you will build it to code.
Then they will want inspections as you go along.
If the contractor is building it, I would have them pull the permit.
Good luck!
Look up the public works department page for the county you live in and they should have a section on how to obtain building permits, what requires a permit, and different types required. If they do not have a listing online, you will have to find the actual government office to acquire the application for a permit.... yellow pages.
easy go to city hall...
You need to take a copy of your plans to the county or town or city planning commission at town hall and submit your info. Then they will issue a building permit that is good for a certain amount of time. Charges vary by locale. You do this so that will have an accurate description of your house come appraisal time. Also, it will have to be inspected at stages to make sure it passes all codes and regulations.
