How do you fix broken concrete patio?

Our patio is old and because of water erosion the individiual slabs have cracked and the wood has rotted away.

Well the best way would be to clear all the old slabs, and rotten timbers, and start afresh. The water if in an cold country will seep in to cracks freeze and expand, thus making the cracks bigger, so before you start make sure you have an game plan that involves any channelling of water away from the wood and preferably the home.
Needless to say it might be quite big job, so work out the finances and make sure you have some planes drawn up.
