Any electrical contractor will be able to assist you in doing this. Check the yellow pages and hire an electrical contractor.

Look into the yellow pages there are so many to choose from
Call a local Electrician Co..They will cut and bend it for you and shouldn't charge to much.
Call some local plumbing companies sometimes they will sell electrical supplies and will usually bend thread the conduit if you purchase at there store. If you bring in the conduit they might have a treading charge.
if you wanted to to it your self the conduit is at home depot. and rent the tools there. other wise call someone to do it in the yellow pages
I would try an Electrical Supply as Electrical conduit is thinner than plumbing pipe and there is a slight difference in the depth of the threading. I had a contractor (US Army Corp of Engineers) Friend of mine put in the saddle.
Some electrical supply houses will even rent you the tools.
Some of the larger construction companies will do it too (for a price), shouldn't cost too much to have it done right.
One other thought, why not use PVC? You can cut it with a hacksaw, glue it, buy the threaded box connectors with locknuts, a couple of 45º bends and couplings will make up a nice custom saddle (the depth depends on the length of the straight piece connecting the 2 45º. It's light and easy to work with and not expensive and you fit everything together before gluing for a perfect fit and it doesn't rust.
I have an idea. If you were to bring the conduit to me at our job site with the measurements, I'd bend, cut and thread it for a 12 pack. Just a thought, call and ask an electrical contractor if that would be possible(call more than one if necessary). Most larger companies couldn't afford the paper work on such a small job and might just do it as good neighborly business. Also don't waste your time with a residential Co. go straight to the commercial Co. they are much more likely to have the equipment.
You only need threads on the end that goes into the meter box hub. The weather head doesn't have threads it clamps on. The 1.5" off set you need can be done with a treated 2x4 cover it with flashing to match the siding an it should look aright.
