check with the people who installed.mushroom grow from spores in the air they should not be there unless they were alredy in the sod try pulling the mushrooms and bagging them . the sunken areas should have soil added to them untill level do it now.

water it
water it
not sure about the fungus but how are you watering the turf, is it possible that the soil underneath the turf where they join are being eroded. If you have a landscaper you should call him because anybody worth their salt will guarantee their work.
grass is generally grown in spent mushroom compost
shrinking is natural give it six months and the joints will go as the new grass grows
If this was a cheap job - then there's your reason. If you paid a fair price with a job guarantee - then call them back in. It's a poor, unprofessional job.
if they are hallucinogenic keep quiet and smoke them if not complain they arent
as for the shrinkage you need to water more the turf is drying out
are the weeds actually mushrooms if so it's probable that the turf was grown on old mushroom compost and some of the spores have survived
check first then eat they will die off soon
if it's other weeds then use a touch weeder to remove them don't use a spray or wait until the lawn is established then use a weed and feed for lawns
I would think you bought some cheap turf if not get them back
