You have two choices. Contact the manufacturer and ask how to obtain one. I'd check to see if you have this covered under the warranty though. For example, my Sunbrella fabric on my patio furniture is warranted for 5 years against, fade, mold and mildew. Or you can have one made. If you chose the later, I would definitely use Sunbrella fabric. I've had pieces out there baking in the sun and 100 degree temps looking like it did when I bought it 8 years ago.
