Are going to remove and replace the pad? Your going to have to clean out and prepare the area first which is the majority of the Labor. You can get a finisher out of the paper or phone book to figure your square footage, and have Ready mix come out and pour it. I think it's a lot easier to just allow professionals do the work. My husband and, I poured a 20 x 40 pad last year for $1500, and now we have a deck around most of it with a garden.

getting ready mix concrete via truck is the best. It's the least messy of all the methods but most expensive. (since you don't have to manage the sand and cement mixing on site)
It's worth it because you get to dump the entire slurry in a single pour...giving you a much even consistency than an on-site mix. Make sure your forms are solid and good to go. You only got about 2 hrs to get the pour done and finished.
Any chance of saving the current pad by putting pavers or tile on top of it? There are a lot of different types of surfacing materials that could be applied on top of an old concrete pad.
