I seem to have a slow but noticeble leak coming from my hot water heater/tank. Can anyone diagnose a solution?

The chances are that its done for and you will need to replace it.
It would have helped if you could have isolated the leak and said where it was at on the tank. lack of info here sorry.
Umm, where is it leaking from?

Could be the drain valve, they tend to leak, try tightening or replacing. Could be the pressure release value, probably would need to be replaced. Could be the base of the tank. Tanks wear out and leak after a while.

It is a pretty easy do-it-yourself project to replace a water heater (esp if it is electric).
How old is your hot water heater?

The harsh reality is that they simply wear out after 10-15 years.

I'm planning to replace mine with a tankless model in the next few months.
If its the element then youll need to replace the water heater.
If it`s getting older 6 + years it`s time to buy a new one. They are not designed to last much longer than that.
You probably have a rust hole in the tank--better get it replaced quickly before it bursts and makes a mess.
