Never use 1/2 you need at least 3/4 and better yet 7/8.
Thicker is better and depending on your distance between floor joists the thicker the board you need. Don't ever let your floor covering be structural so you don't get squeaks or bows in the floor. Be sure to use sub floor adhesive and screws of proper length in the floor.

NO. Your BEST INSTALL is to use 3/4 X 3in or 4in white wood or better boards running diagonally from the direction of the floor you will be laying. This way the floor will be able to move and breath along with the home. If your going to lay your floor in this direction -------- then your sub floor should run this ///// way. If you must use plywood then the sub-floor needs to be installed opposite of the way your finished wood will be installed. In other words your sub floor is laid like this IIIIIII then your finished floor needs to run like this----------------. Also if you use plywood I would use a bonding agent such as PL200 or Hard as nails along with nailing down the sub floor. DO NOT GLUE THE UNDERSIDE OF YOUR HARD WOOD FLOOR.
