There are hangers in the wall to hold the toilet up. Between these hangers is the sewer pipe and connection that the toilet hooks to when you hang it. You will have to remove the hanger and the sewer pipe in the wall. Then you will have to measure out and find where the new toilet will go. You will need to be 12"-12 1/2" out from the FINISHED wall to set the new toilet. Drill a hole through the floor, set the stool flange in the hole, connect the sewer to the flange and the other end to the existing sewer in the house. You will then have to get a new supply pipe for the water to the new toilet. It'll take a while, but it's not real hard to do. Good luck!

Pretty major job, just did this about a month ago at my brothers house. After unbolting the old toilet from the wall, we cut off the pvc waste pipe under the house & ran new pvc for the new toilet & installed the flange.
Also had to patch in the wallboard.
