Here are a few suggestions, some of which you may not have considered:
1. Yellow Pages, under 'doors.' Or ads in the local paper.
2. Salvage or reclamation yards, again listed in Yellow Pages. Of course, it is a matter of pot luck, but you might find exactly the right door at a fraction of the price you would pay to have one made.
3. Sunderland too far for you? If not, try Withknobson.com - they show kitchen doors on their website.
4. Try lateral thinking: if a door of those exact dimensions proves very expensive and/or difficult to get, look for old shutters (we're talking salvage yards again). Or be prepared to find a door as close to those measurements as you can, and then build a new wider/narrower/taller/shorter doorframe around it. You could save yourself a lot of money that way.
