First you need to prioritize, I would start by making a list of things that need to be changed/organized first and once you have your list you need to make a schedule of your daily acitvities and once that is complete you need to focus on what you are wasting time on and cut some of the waste out. You need to stick with your schedule, and you made it so you should be able to follow up with your own requests. You just have to do it, bottom line if you don't want to your life will not change.

You have to genuinely find it within you to make the change. It's not just about doing things it's about changing your lifestyle and attitude to doing things. Make a serious step by doing your best to get things done. List a plan of action to get into regarding your house and your office during your free time- any free time to organise things. Once you feel comfortable about your home environment and office, you will feel great really and happy. Regarding dieting, see your doctor . She will give you a diet plan you can work on.
